Case Management

Case Management is a collaborative process that includes assessment, planning, facilitation, care coordination, evaluation, and advocacy on behalf of our community members. Our case managers assist individuals and families in achieving wellness and autonomy through individual sessions and workshops that are culturally and linguistically accessible and trauma informed.

  • Benefits Assistance

    • Assistance applying for and managing Illinois Department of Human Services benefits

      • Medicaid

      • SNAP - Supplemental Nutrition Assistance Program

      • TANF - Temporary Assistance for Needy Families

    • Eligibility information and counseling

    • Referrals for WIC - Women, Infants, and Children

    • Supplemental Security Income (SSI) and other related SSA matters

    • Administrative assistance with issues at local, state, and federal government offices

    • Understanding and responding to traffic and parking citations

    • Accident reporting

    • Referrals for legal representation

    • Assistance with jury summons

    • Help with tenant and housing issues

    • State ID/License renewal

    • Gaining accessible parking

    • Insurance provider communication, including changing providers and maneuvering coverage

    • Scheduling doctor’s appointments per client’s request

    • Mental health referrals

    • Workshops and outreach on public and personal health issues

  • Employment

    • Connecting to existing employer partners

    • Assistance with job application completion
